    Medical certificate form (in order to obtain a work permit and/or a visa long stay):

     

    Please contact the Embassy for the name and the address of an acknowledged doctor.

    Legalization procedure for Romanians:

     

    In case the medical certificate is used to obtain a work permit in Belgium: the form has to be presented to this Embassy for legalization, after having been filled out by the Romanian doctor (please provide us with the data of the doctor – professional address + telephone number of the practice).

     

    In case the medical certificate (annexe IX) is used to obtain a visa long stay: after the form has been filled out by a Romanian doctor, this certificate has to be legalised by the local Public Health Office, and then an apostille needs to be added by the competent Romanian “Prefectura de Judet”.

     

     

    Legalization procedure for Moldavians:

     

    In case the medical certificate is used to obtain a work permit in Belgium: the form has to be presented to this Embassy for legalization, after having been filled out by the Moldavian doctor (please provide us with the data of the doctor – professional address + telephone number of the practice).

     

    In case the medical certificate (annexe IX) is used to obtain a visa long stay: after the form has been filled out by a Moldavian doctor, an apostille has to be put on it by the Moldavian Ministry of Foreign Affairs.
