The complete file should be lodged in 3 exemplars : 1 original and 2 copies
1. Personal appearance of the applicant with appointment at
1 909 1014545 (PLDT only through landline or SMART mobile phone)

2. Passport valid at least 12 months from the date the application is submitted (**)
3. Two duly completed and signed application forms with two recent 2 x 2 coloured passport-size photos with WHITE background
4. Birth Certificate of the applicant on NSO Security Paper, authenticated by DFA
5. Copy of the Identity Card (carte d'identité belge/Belgische identiteitskaart) of the spouse residing in Belgium who is a Belgian citizen or an EU national
6. Proof of sufficient lodging in Belgium
7. Proof of regular, sufficient and stable income EU partner
8. Proof of medical insurance covering both the applicant and the EU partner
SPOUSE :
1. Marriage Certificate of the applicant on NSO Security Paper, authenticated by DFA
2. Proof of Civil Status of the applicant before marriage
· If single, Certificate of Single Status called Certificate of Non-Appearance in the National Indices of Marriage on NSO Security Paper authenticated by DFA
· If annulled/divorced, Annulment/Divorce Certificate on NSO Security Paper authenticated by DFA
· If widow/widower, Death Certificate on NSO Security Paper authenticated by DFA

DEPENDENTS 21 years old and under:
1. Birth Certificate of the applicant on NSO Security Paper, authenticated by DFA
2. If 18 years old and above, Certificate of Single Status called Certificate of Non-Appearance in the National Indices of Marriage on NSO Security Paper authenticated by DFA
3. If 18 years old and above, NBI Clearance
DEPENDENTS over 21 years old:
1. Birth Certificate of the applicant on NSO Security Paper, authenticated by DFA
2. Certificate of Single Status called Certificate of Non-Appearance in the National Indices of Marriage on NSO Security Paper authenticated by DFA
3. Proof of financial dependency
4. Last Tax Declaration of the parent/s to be joined in Belgium
5. Proof that the applicant has no income (BIR Certification + SSS Certification)
ASCENDANTS (Parents) - ONLY possible FOR ascendants of EU & EEE partners
1. Birth Certificate of the applicant on NSO Security Paper, authenticated by DFA
2. Proof of Civil Status of the applicant:
a. If Married, Marriage Certificate of the applicant on NSO Security Paper, authenticated by DFA
b. If single, Certificate of Single Status called Certificate of Non-Appearance in the National Indices of Marriage on NSO Security Paper authenticated by DFA
c. If annulled/divorced, Annulment/Divorce Certificate on NSO Security Paper authenticated by DFA
d. If widow/widower, Death Certificate on NSO Security Paper authenticated by DFA
3. Birth Certificate of the son/daughter to be joined in Belgium on NSO Security Paper, authenticated by DFA
4. Proof of financial dependency
5. Last Tax Declaration of the son/daughter residing in Belgium who is a Belgian citizen or an EU national
6. Proof that the applicant has no income (BIR Certification + SSS Certification) and family composition (Barangay)
No documents should be older than 6 months, for the Embassy of Belgium it is not necessary to submit a translation of any documents drafted in English or any of our Belgian national languages.
(**) Please be advised that a married woman, in adopting a surname, may elect any of the following three (3) options provided in Article 370 of the Civil Code, namely: (1) her maiden first name and surname and add her husband’s surname; or (2) her maiden first name and her husband’s surname; or (3) her husband’s full name, but prefixing a word indicating that she is his wife, such as “Mrs.”. Government-issued IDs such as passport and driver’s license, and bank statements, and other similar proof of identification and records may evidence use of any of the allowable surnames by a married woman.
IMPORTANT
- The Embassy of Belgium does not require a plane ticket to be submitted when lodging the application. Any agent/intermediary/applicant pretending the contrary is defrauding you. Unless otherwise specified a simple reservation is enough.
- The Embassy of Belgium does not accept to work with any au pair or travel agency (be it Foreign or Filipino) Agencies who claim so are giving fraudulent information.
- The Embassy of Belgium does not require any other consular fees than mentioned on this webpage CONSULAR FEES. Any agent/intermediary/applicant pretending the contrary is defrauding you.
- There is a distinct possibility that the applicant will be invited for a personal and in-depth interview with the visa officer at a later date than the lodging of the application
- The average processing time by the Embassy of this type of applications is 6 weeks. If the application is forwarded to Home Office for final decision one should take into account 1 to 2 months on average extra processing time.
